Fortay PS3 Application Instructions:

Items in RED are required, Items in BLUE are optional.

  • Please answer all questions honestly.
  • Please create a New Topic, with the title"Your Name App"
  • Copy and paste the following APP FORM into your new topic
  • Delete answers as appropriate
  • Remember to add the senior members from the member list, for quick application approval.
  • Now add your 4TaY clan tag and come online for the Senior members to introduce you to the rest of the clan.
